Bell cleared to buy Astral Media, creates a Canadian TV powerhouse
Bell tried to shake up the Canadian media landscape last year by acquiring Astral Media, but it ran into a CRTC-sized roadblock -- regulators didn't want 25 TV stations moving to one provider. After some big concessions, however, Bell has received approval to buy Astral for $3.2 billion. The revised deal gives Bell control of 12 channels that include The Movie Network, HBO Canada's owner. Bell is offloading some important TV content to move forward, though. Corus gets several recognizable channels that include the Cartoon Network and Teletoon, while big stations like Disney XD and MusiquePlus are on the auction block. Not that Bell will complain too loudly when the buyout closes on July 5th, mind you. The merger still gives it 35.8 percent of the English Canadian TV market and 22.6 percent of its French Canadian equivalent, or enough to immediately eclipse rivals like Rogers and Quebecor.

American student killed in Egypt violence taught English
ALEXANDRIA, Egypt (Reuters) - An American college student stabbed to death during a protest in Egypt was in the country teaching English to children and improving his Arabic, according to a Facebook post on Saturday that appeared to be from his family.
Andrew Pochter, 21, from Chevy Chase, Maryland, died after being stabbed in the chest in the coastal city of Alexandria, where anti-government protesters stormed an office of the ruling Muslim Brotherhood.
It was not clear what Pochter was doing at the protest, but Egyptian officials said he was carrying a small camera.
A statement on a Facebook page entitled "R.I.P Andrew Driscoll Pochter", which appeared to have been posted by his family, said Pochter had travelled to Alexandria for the summer to teach English to 7- and 8-year-old Egyptian children and to improve his Arabic.
The page had also been posted on by colleagues of Pochter at the U.S. educational non-profit organization where he was working.
The family statement read: "He went to Egypt because he cared profoundly about the Middle East, and he planned to live and work there in the pursuit of peace and understanding."
Pochter was looking forward to beginning his junior year at Ohio's Kenyon College and had planned to study abroad in Jordan next spring, according to the statement. He had also spent time in Morocco.
"Andrew was a wonderful young man looking for new experiences in the world and finding ways to share his talents while he learned," it said.
A statement from Kenyon College said Pochter was interning in Alexandria with AMIDEAST, an American non-profit organization that runs education and development programs in the Middle East and North America.
A State Department spokeswoman confirmed that Andrew Pochter was killed on Friday in Alexandria.
"We extend our deepest condolences to his family and friends," Marie Harf said. Harf said the U.S. embassy in Cairo and the State Department's Bureau of Consular Affairs were providing "appropriate consular assistance."
The Muslim Brotherhood said eight of its offices had been attacked on Friday, including the one in Alexandria. Officials said more than 70 people had been injured in the clashes in the city, adding to growing tension ahead of mass rallies on Sunday aimed at unseating President Mohamed Mursi.

Dream final: Host Brazil vs world champion Spain
RIO DE JANEIRO (AP) ? The Confederations Cup has the final many wanted: a long-awaited matchup between world champion Spain and host Brazil.
The most dominant national team in recent years and the most successful team ever in international play will meet Sunday at Maracana Stadium for the title of the eight-nation warmup tournament for next year's World Cup.
It will be the first meeting between the nations since 1999 and their first competitive match since Brazil's 1-0 win on Socrates' goal in the first round of the 1986 World Cup in Mexico.
"It's the match everyone wanted to happen," said 21-year-old Brazilian forward Neymar, the star of the five-time world champions. "The entire world wanted it and everybody will be watching it."
With more than 70,000 Brazilian fans packing the iconic venue, Brazil will be seeking its third straight Confederations Cup title. Spain enters unbeaten in a world record 29 competitive matches over three years since losing its 2010 World Cup opener to Switzerland.
"For us it's a dream game," Spain coach Vicente Del Bosque told FIFA.
Brazil was eliminated in the quarterfinals in the last two World Cups and hasn't won a significant title since the 2009 Confederations Cup in South Africa. Spain won the 2010 World Cup along with the 2008 and 2012 European Championships.
"They are the current world champions, they have to be praised," Brazil captain Thiago Silva said. "But anything can happen in a final, and I'm certain that Brazil will be fully prepared for the matchup."
Spain hasn't lost in 26 matches overall since a 1-0 defeat against England at Wembley in 2011. Brazil struggled after coach Luiz Felipe Scolari replaced Mano Menezes in November, winning only one of its first six matches. Scolari, who coach Brazil to the 2002 World Cup title, enters the final with a five-game winning streak.
"There is no doubt it will be an even match," Brazil right back Daniel Alves said Saturday. "There is mutual respect between these two national teams."
Brazil beat Japan, Mexico and Italy in the group stage before defeating South American champion Uruguay 2-1 in the semifinals. Spain defeated the Uruguayans in its opener, then routed Tahiti and beat Nigeria before getting past Italy on penalty kicks in the semifinals.
"It's the match everybody has been waiting for," said Brazil assistant coach Carlos Alberto Parreira, who led the national team to the 1994 world title. "We all know how good Spain is, but Brazil is going through a sensational moment and is full of confidence."
While the host hasn't played since Wednesday, Spain endured overtime and the shootout in the semifinal against Italy in the heat of Fortaleza on Thursday. Del Bosque has said Spain will not use fatigue as an excuse, and Scolari also downplayed the issue.
"They were able to rest all of their starters when they played Tahiti, so basically they had to play one game less than we did," he said.
Both coaches will have all of their top players available for the match at the renovated Maracana.
Four players who will be in Sunday's final were nominated for the Golden Ball award handed to the best player at the Confederations Cup: Brazil's Neymar and Paulinho and Spain's Andres Iniesta and Sergio Ramos.

ST. PETERSBURG, Fla. (AP) ? Max Scherzer is getting as good at deflecting attention from his accomplishments as he is at shutting down opposing hitters.
The Detroit right-hander became the first pitcher to win 12 consecutive decisions to begin a season in 27 years on Friday night, riding a pair of home runs by Miguel Cabrera and a mammoth shot by Prince Fielder to beat the Tampa Bay Rays 6-3 and take over the major league lead in victories.
"It's really nice to be 12-0," said Scherzer, who allowed three runs, four hits, walked one and struck out nine in seven innings to become the first pitcher in Tigers history to win his first 12 decisions.
"I'm pitching well, but the reason I'm 12-0 is because of my offense," he added. "You got to see firsthand today the best player in the game hitting two home runs on three pitches and going 4 for 4. And Prince hits a bomb. It's the offense that set me up."
Scherzer became the first in the majors to begin a season with at least 12 straight victories since Roger Clemens did it on the way to starting 14-0 with the Boston Red Sox in 1986. He's 2-0 with a 2.57 ERA and 18 strikeouts in two wins over Tampa Bay this season.
"He's just good. He's got a little of that whiffle ball look from the side, where the ball is jumping all over the place," Rays manager Joe Maddon said. "Then he gets the velocity when he wants it. There's a reason why he's 12-0. He's very good."
It helps to have Cabrera in the middle of the lineup.
The 2012 AL MVP went 4 for 4 and drove in three runs, boosting his major league-leading batting average to .377 with 24 homers and 81 RBIs, also tops in the big leagues. He hit a two-run homer in the first inning off Alex Colome (1-1) to extend his hitting streak to a season-best 13 games, then added a solo shot off the rookie for a 3-0 lead in the fourth.
Cabrera also singled in the sixth, giving him three of Detroit's four hits off Colome. Fielder doubled in the fourth, tagged up and hustled to third base on Victor Martinez's fly ball to right and eventually scored on a wild pitch to make it 4-0 before the Rays gradually climbed back into the game.
Ben Zobrist and Wil Myers hit solo homers for Tampa Bay. Luke Scott's RBI double trimmed Scherzer's lead to 4-3 in the seventh.
"All I thought about is winning today. My personal record is more a reflection of the team," Scherzer said. "I don't get caught up in the win-loss record because it's kind of fluky. Yesterday Doug (Fister) goes seven innings, one run and gets a no-decision. I got six and (allow) three and get a win, so that's why it's a fluky stat."
Cabrera doubled off Kyle Farnsworth in the eighth for his fourth hit. Fielder gave the Detroit bullpen some breathing room when he followed with his first homer since June 4, a towering two-run shot off Cesar Ramos that struck one of the catwalks that support the roof at Tropicana Field.
The Rays estimated Cabrera's home runs traveled 388 and 432 feet. Fielder's drive, which bounced off the catwalk and rolled back toward the infield, was estimated at 414 feet.
Cabrera, who leads the majors in hits and RBIs and is second behind Baltimore's Chris Davis in home runs, was asked how far Fielder's homer might have gone if it hadn't hit something.
"Miami," Cabrera said. "It was very far. It was impressive."
Al Alburquerque and Drew Smyly worked a scoreless eighth for the Tigers. Joaquin Benoit finished a combined five-hitter, earning his fifth save.
Scherzer, who's fanned at least six in each of his 16 starts this season, retired 11 in a row before Zobrist's first-pitch drive to right with two outs in the fourth. The closest the Rays had come to getting a hit up until then was Evan Longoria's second-inning grounder over the mound that Omar Infante ran down behind second base before making an off-balance throw to first for the out.
Longoria was removed from the game following that inning. The Rays later announced he irritated plantar fasciitis in his right foot, which has bothered him for the past month.
Maddon said Longoria, who has 17 homers and 47 RBIs, will not play Saturday and is doubtful for Sunday.
"A little bit tender in the foot area. We have to wait for it to calm down (Saturday) to make a better evaluation," the Tampa Bay manager said. "Of course I'm concerned, but I don't know the level yet until I get more information."
Notes: Reigning AL Cy Young winner David Price will rejoin Tampa Bay's rotation Tuesday night at Houston. The left-hander went on the disabled list for the first time in his career May 16 with a strained left triceps. He made two minor league rehab starts with Class A Charlotte and said he felt good after a bullpen session Friday. The Rays have gone 21-20 while he's been on the DL. ... Farewell, Alec Baldwin's Twitter account. Again. After his wife Hilaria was accused by London's Daily Mail of tweeting a variety of comments during the hours the pair were attending James Gandolfini's funeral Thursday, he went on a rage-filled rampage against the author of that article, with comments like "I am gonna f--- you up."
In the article, writer George Stark noted that Hilaria's feed was full of "upbeat" posts, talking about wedding anniversary presents and a summer smoothie she enjoyed after seeing it on "Rachael Ray."
But the reporter may need a refresher course in Twitter time-stamp interpretation; according to a statement released Friday morning by the Baldwin camp, the reporter misinterpreted when the tweets were sent.
The statement read: "It's disgraceful that a reporter would manufacture and publish a story and not call for comment or explanation -- especially when it's intended to take away attention from honoring the memory of a beloved figure like James Gandolfini. There are multiple witnesses to the fact that Hilaria left her phone in the car, and wouldn't, couldn't and didn't tweet during the service.... And as for the tweets -- 1) some of the time stamps in the report were wrong because he listed the time of the original tweet not the retweet 2) she left early and wasn't in the service at the time of tweeting 3) it's possible that a previously scheduled tweet wasn't cancelled."
Still, Baldwin wasn't in the mood for a rational discussion, and his responsive tweets (now deleted, along with that account) grew increasingly irate. As noted in a report from TODAY's Mara Schiavocampo, Baldwin tweeted:
"Someone wrote that my wife was tweeting at a funeral. Hey. That's not true. But I'm gonna tweet at your funeral," he said.
He also wrote, "My wife and I attend a funeral to pay our respects to an old friend, some toxic Brit writes this (expletive) trash."
And this: "I'm gonna find you, George Star, you toxic little (expletive), and I'm gonna f--- you up."
Baldwin's temper on and off Twitter has been the subject of headlines before; in 2011 he was asked to stop playing "Words With Friends" on a flight and was ejected from the plane; in 2012 he got in a scuffle with a photographer (another Twitter rant followed).
Meanwhile, though Baldwin has quit Twitter again, his wife Hilaria has continued to post. One of her latest? "I would like a real apology."

Facebook to pull ads from pages with sex, violence

Facebook says it will no longer allow ads to appear on pages with sexual or violent content, as the online social network moves to appease marketers being associated with objectionable material.
The announcement on Friday came a month after several businesses pulled their ads from Facebook amid reports of pages on the site that promoted violence against women.
Facebook said at the time that it needed to improve its system for flagging and removing content that violated its community standards, which forbid users from posting content about hate-speech, threats and pornography, among other things.
Ads account for roughly 85 percent of revenue at Facebook, the world's largest social network with 1.1 billion users. Facebook said the changes would not have a meaningful impact on its business.
On Friday, Facebook said it also needed to do more to prevent situations in which ads are displayed alongside material that may not run afoul of its community standards but are deemed controversial nonetheless.
A Facebook page for a business that sells adult products, for example, will no longer feature ads. Previously such a page could feature ads along the right-hand side of the page so long as the page did not violate Facebook's prohibition on depicting nudity.
The move underscores the delicate balance for social media companies, which features a variety of unpredictable and sometimes unsavory content shared by users, but which rely on advertising to underpin their business.
"Our goal is to both preserve the freedoms of sharing on Facebook but also protect people and brands from certain types of content," Facebook said in a post on its website on Friday.
Facebook said that it would expand the scope of pages and groups on its website that should be ad-restricted and promised to remove ads from the flagged areas of the website by the end of the coming week.
Pages and groups that reference violence will also be off limits to ads, the company said. A Facebook spokeswoman noted that the policy would not apply to the pages of news organizations on Facebook.
Facebook said the process of flagging objectionable pages and removing ads would initially be done manually, but that the company will build an automated system to do the job in the coming weeks.

Alleged press images of Sony Xperia i1 Honami leak out
After posing for live shots yesterday, the Sony Xperia Honami made yet another appearance. This time around, the yet to be revealed top-shelf smartphone showed up in alleged press images, reportedly taken from a Xperia i1 Honami system dump.
The press images show both what appears to be the international version of the smartphone, as well as one which is bound for NTT Docomo in Japan. The press renders of the former appear to be in line with the recently leaked live images of the device. Sadly, no images of the device's back are available at the moment.
There is no official launch date for the Sony Xperia Honami. Considering the intensifying leaks of the device however, we doubt it will be a long wait. Until then, use the usual pinch of salt when checking out the images.

S. African president: Prayers continue for Mandela
PRETORIA, South Africa (AP) ? South Africa's president says the nation continues to pray for Nelson Mandela's health and well-being as the 94-year-old beloved leader remains hospitalized in critical condition.
President Jacob Zuma made the comments Saturday during a press conference with visiting President Barack Obama.
Zuma called Mandela the "founding president of our democracy who is much loved by our people and the world."
The South African government has said that Mandela's condition is stable but remains critical. He was admitted to a Pretoria hospital three weeks ago with a recurring lung infection.
Obama plans to visit privately Saturday with Mandela's relatives, but doesn't intend to see the man he has called a "personal hero."
Mandela became South Africa's first democratically elected president in 1994 after spending 27 years in prison.

Apparently with an eye on rumors of Apple entering the video game console business (again), Google is reportedly working on their own Android-based console. Oh, and an Android watch, too, which will also probably have an Apple competitor. ?

According to the report from the Wall Street Journal?(based, of course, on unnamed sources), one of the rumored devices ? which also includes a Nexus Q media-playing device ? would be released this fall. But according to the Journal, it looks like the company might have another compelling reason for getting into the game on gaming, at least when it comes to competing with more traditional gaming console makers: ?
"Games that run on Android software have proved particularly popular, and they are growing much more quickly than games made for the big-name consoles supplied by Microsoft Corp., Sony Corp. and Nintendo Co. The appeal of such games has prompted the development of new devices aimed specifically for Android by other hardware companies."?
Google Play's store has over 100,000 games already loaded, so the company wouldn't be hurting for content if they did launch a console. They wouldn't, however, be the first Android-based console out there. The $99 Ouya, which launched this week, runs a version of Android.?

LONDON (Reuters) - Starbucks, whose thin tax payments in Britain provoked a backlash against corporate tax avoidance when revealed by Reuters, paid no tax for the year to September 30, 2012.
The coffee giant's main UK subsidiary reported its 15th straight annual loss at its UK stores in accounts filed on Friday.
Reuters revealed in October that Starbucks reported consistent UK losses while telling investors the British unit was profitable and promoting managers of the unit within the group.
Friday's accounts showed a UK loss of 30 million pounds ($46 million), down from the 32 million pounds loss it reported for the previous year, helped by a 4 percent rise in turnover to 413 million pounds.
The company cited challenging economic conditions and a competitive UK coffee market, although the accounts show profits were also undermined by a royalty for the use of the Starbucks brand of 26 million pounds.
This is paid to an affiliate in the Netherlands, where Starbucks has negotiated what it said was a very low tax rate.
The UK unit also paid 2 million pounds in interest to affiliated companies, the accounts showed.
Following widespread criticism from politicians and the picketing of stores, Starbucks said it would pay or pre-pay around 10 million pounds a year in taxes in 2013 and 2014.
The company said it would not take tax deductions for certain intercompany payments such as the royalty fee, interest payments and the 25 percent mark up on coffee beans that is paid to a Swiss-based Starbucks coffee purchasing unit.
Starbucks recently paid 5 million pounds to the UK tax authority as the first installment of its 2013 tax bill, British media reported earlier this month.
Starbucks declined immediate comment.

Stocks gain on encouraging news on the US economy
NEW YORK (AP) ? Stocks are closing higher on Wall Street after investors got a double dose of good news about the U.S. economy.
The Dow Jones industrial average rose 114 points, or 0.8 percent, to close at 15,024 Thursday. It was the third straight triple-digit gain for the Dow. The Dow is still down slightly for the month.
The Standard & Poor's 500 index was up nine points, or 0.6 percent, at 1,613.
The Nasdaq rose 25 points, or 0.8 percent, to 3,401.
Consumer spending rose last month as incomes increased. Claims for unemployment benefits fell last week.
The yield on the 10-year Treasury note fell to 2.48 percent.
Five stocks rose for every one that fell on the New York Stock Exchange. Volume was lighter than average, 3.3 billion shares.

Vatican monsignor arrested in 20M euro plot
VATICAN CITY (AP) ? A Vatican cleric and two other people were arrested Friday by Italian police for allegedly trying to smuggle 20 million euros ($26 million) in cash into the country from Switzerland by private jet. It's the latest scandal to hit the Holy See and broadens an Italian probe into its secretive bank.
Monsignor Nunzio Scarano, already under investigation in a purported money-laundering plot involving the Vatican bank, is accused of corruption and slander and was being held at a Rome prison, prosecutor Nello Rossi told reporters.
Scarano's arrest came just two days after Pope Francis created a commission of inquiry into the Vatican bank to get to the bottom of the problems that have plagued it for decades and contributed to the impression that it's an unregulated, offshore tax haven.
Prosecutor Rossi said the Swiss operation involved three people, all of whom were arrested Friday: Scarano, a recently suspended accountant in the Vatican's main finance office, Italian financier Giovanni Carenzio, and Giovanni Zito, who at the time of the plot was a member of the military police's agency for security and information.
Rossi detailed a remarkable plot ? uncovered by telephone wiretaps ? in which the three allegedly planned to bring into Italy some 20 million euros in cash that financier Carenzio held in his name in a Swiss bank account without paying customs at the airport, as would be required.
Scarano's attorney, Silverio Sica, said his client was something of a middleman: The 20 million euros belonged to friends who had given the money to Carenzio to invest but wanted it back. The plot would presumably enable them to avoid paying customs fees or having any paper trail of such a large amount of money entering Italy.
Rossi identified the friends as members of the Italian shipping family d'Amico and suggested that the money was being held in Switzerland to avoid paying Italian taxes. An email seeking comment from the family's Rome-based company, the d'Amico Societa di Navigazione SpA, wasn't immediately returned.
According to prosecutors, Zito, the agent, called in sick to his job one day in July 2012, rented a private plane and flew with Carenzio to Lucerne, Switzerland. There, Carenzio was supposed to withdraw the cash from his bank account and hand it over to Zito to bring back to Italy. The plan was so detailed there was even to be a car and driver waiting at the airport to bring the money to Scarano's apartment in Rome, Rossi said.
The money could have been transported relatively easily because euros are issued in high denominations. If the cash had been withdrawn in the largest denomination ? 500 euro notes ? it would have weighed 44 kilograms (97 pounds) and fit in a suitcase.
But at a certain point in Lucerne, the deal fell through and Carenzio made excuses that the bank couldn't come up with the money, Rossi said. He declined to identify the bank.
Zito returned to Rome empty-handed but still demanded from Scarano his fee of 600,000 euros for the operation. Scarano cut him one check for 400,000 euros which he deposited. He gave him a second check for 200,000 euros, but in a bid to prevent the check from being deposited, reported it as missing, the prosecutor said.
That put a block on the check and resulted in Scarano being accused of slander for filing a false report knowing that the check was in Zito's hands, Rossi said.
Scarano, as well as the other two, are also accused of corruption. If they are indicted and convicted, they could face up to five or six years in prison, prosecutors said.
Sica, the lawyer, said Scarano had no immediate comment on the charges.
The Vatican bank, known as the Institute for Religious Works, or IOR, is cooperating with Italian authorities and its lay board has launched an internal investigation, spokesman Max Hohenberg said.
Rossi, the Italian prosecutor, described the operation as one branch in a "mosaic" of investigations targeting the IOR, which has long been a source of scandal for the Holy See. That said, the Swiss investigation didn't immediately appear to directly involve the IOR.
The checks Scarano wrote to Zito, for example, came from an Italian bank account, prosecutors said. They declined to say if Scarano received any payment for his role in the plot, or if his IOR account was used at all.
Rossi's team of prosecutors in 2010 placed the top two Vatican bank officials under investigation for allegedly violating anti-money laundering norms during a routine transaction involving an IOR account at an Italian bank. They ordered the 23 million euros in the transaction seized. The money was eventually unfrozen but the two men remain under investigation.
Rossi's team is also working with prosecutors in Salerno on a separate money-laundering investigation involving Scarano and his IOR account.
According to Sica, the lawyer, Scarano took 560,000 euros ($729,000) in cash out of his IOR bank account in 2009 and carried it out of the Vatican and into Italy to help pay off a mortgage on his Salerno home.
The money had come into Scarano's IOR account from donors who gave it to the prelate thinking they were funding a home for the terminally ill in Salerno, Sica said.
To deposit the money into an Italian bank account ? and to prevent family members from finding out he had such a large chunk of cash ? he asked 56 close friends to accept 10,000 euros apiece in cash in exchange for a check or money transfer in the same amount. Scarano was then able to deposit the amounts in his Italian account.
The lawyer said Scarano had given the names of the donors to prosecutors and insisted the origin of the money was clean, that the transactions didn't constitute money-laundering, and that he only took the money "temporarily" for his personal use.
The home for terminally ill was never built, though the property has been identified, Sica said.
The Vatican spokesman, the Rev. Federico Lombardi, said Scarano was suspended last month and that the Vatican was taking the appropriate measures to deal with his case.
Francis has made clear he has no tolerance for corruption or for Vatican officials who use their jobs for personal ambition or gain. He has said he wants a "poor" church and a church that is for the poor, one that goes out to the "peripheries" to minister to those most needy. He has also noted, tongue in cheek, that "St. Peter didn't have a bank account."
On Wednesday, he named five people to head a commission of inquiry into the Vatican bank's activities and legal status "to allow for a better harmonization with the universal mission of the Apostolic See."

Elevated HOXB13/IL17BR ratio may indicate who should receive extended treatment with letrozole
A biomarker reflecting expression levels of two genes in tumor tissue may be able to predict which women treated for estrogen-receptor (ER)-positive breast cancer should receive a second estrogen-blocking medication after completing tamoxifen treatment. In their report being published online in the Journal of the National Cancer Institute, Massachusetts General Hospital (MGH) Cancer Center investigators describe finding that the HOXB13/IL17BR ratio can indicate which women are at risk for cancer recurrence after tamoxifen and which are most likely to benefit from continuing treatment with the aromatase inhibitor letrozole (Femara).
"Most patients with early-stage, ER-positive breast cancer remain cancer-free after five years of tamoxifen treatment, but they remain at risk of recurrence for 15 years or longer after their initial treatment," says Dennis Sgroi, MD, of the MGH Cancer Center and Department of Pathology, lead and corresponding author of the report. "Our biomarker identifies the subgroup of patients who continue to be at risk of recurrence after tamoxifen treatment and who will benefit from extended therapy with letrozole, which should allow many women to avoid unnecessary extended treatment."
Previous research by Sgroi's team, in collaboration with investigators from bioTheranostics Inc., discovered that the ratio between levels of expression of two genes HOXB13 and IL17BR in tumor tissue predicted the risk of recurrence of ER-positive, lymph-node-negative breast cancer, whether or not the patient was treated with tamoxifen. The current study of patients from MA.17, the highly successful clinical trial of letrozole, was designed to evaluate the usefulness of the HOXB13/IL17BR ratio for both prognosis predicting which tamoxifen-treated remained patients at risk of recurrence and for identifying who could benefit from continued treatment with letrozole.
To answer those questions the investigators analyzed primary tumor samples and patient data from the placebo-controlled MA.17 trial, which confirmed the ability of extended letrozole therapy to improve survival after the completion of tamoxifen treatment. Tissue samples were available from 83 patients whose tumors recurred during the study period 31 who had received letrozole and 52 in the placebo group and 166 patients with no recurrence, 91 of whom had received letrozole, with 75 getting the placebo. Analysis of the tumor samples revealed that a high HOXB13/IL17BR ratio meaning the expression level of HOXB13 is greater than that of IL17BR predicts an increased risk for tumor recurrence after tamoxifen therapy, but that elevated risk drops significantly if a patient receives letrozole
Paul E. Goss, MD, PhD, director of the Breast Cancer Research Program at the MGH Cancer Center and a co-author of the report, explains, "This discovery means that about 60 percent of women with the most common kind of breast cancer can be spared unnecessary treatment with the concommitant side effects and costs. But more importantly, the 40 percent of patients who are at risk of recurrence can now be identified as needing continued therapy with letrozole, and many will be spared death from breast cancer." He and Sgroi note that their findings need to be validated by additional studies before they can be put into clinical practice.
